Graduate School Forms: 

Dates and forms are subject to change as determined by the School of Graduate Studies.  All forms are completed through the following address:


2nd Year 

  • Committee Appointment Form - Completed during semester of the Candidacy Review which takes place after student has completed 24cr, initiated by Student


3rd Year 

  • Application for Graduation - Initiated by Student two semesters prior to graduation

  • Manuscript Review Form - Three signatures, Initiated by Student, that all committee members sign to certify that a thesis is fully approved and ready for the Graduate School review

  • Notification of Thesis/Dissertation Defense - deadline is 10 days prior to orals and will include abstract of thesis, initiated by Committee Chair


  • Outside Observer Narrative Report  - Initiated by Outside Observer after orals


  • Results of Thesis/Dissertation Defense - Initiated by Committee Chair after orals


  • Result of Culminating Experience - Initiated by Graduate Coordinator after orals
